Every employer and self-employed person must assess the risks of their work. With 5 or more employees, you must write down the significant findings.

Not named in the law, but main contractors usually ask for one before you start. It shows how the job will be done safely, step by step.
HSE: construction ↗CDM 2015
The Construction (Design and Management) Regulations apply to all building work, including jobs for homeowners, and set duties for clients, designers and contractors.
